Knowledge, skills, and abilities
- Must be a registered nurse (RN)
- Licensed practical nurses (LPNs) and licensed vocational nurses (LVNs) may be hired in secondary NAC positions with RN oversight
- MDS experience; RAC-CT is preferred
- At a minimum, must have knowledge of current state and federal regulations, particularly the following pertinent sections of Appendix PP:
- §483.20 Resident Assessments
- §483.21 Comprehensive Resident-Centered Care Plans
- Attention to detail and accuracy in MDS coding
- Have the ability to learn computer systems, system applications, and related office equipment
- Must be CPR certified
PERFORMACE REQUIREMENTS
Essential Functions:
- Coordinate the completion of the comprehensive care plan according to regulatory requirements.
- Be highly involved in determining skilled level of care for Medicare residents and procuring required Medicare-specific documentation; be responsible for physician certification/recertification of a skilled level of care throughout the Medicare stay; be involved in making Medicare eligibility determinations.
- Audit and improve staff education/competency as needed to ensure accurate and timely completion of supporting documentation and MDS assessments
- Plan, organize, prioritize, work independently, and meet deadlines
- Provide insight and analysis of MDS-based Quality Measures
- Serve as a member of the quality assessment and assurance (QAA) and/or the quality assurance and performance improvement (QAPI) committees
- Foster effective working relationships and build consensus
- Work effectively with individuals at all levels of the organization, as well as with residents, family members, visitors, government personnel, and the public
- Be knowledgeable of federal regulations, practice standards, and procedures, as well as guidelines pertaining to the RAI process
- May be required to administer medications as ordered
- Make rounds with physicians as needed
- Assist with regulatory surveys; yearly and PRN
- Participate in oversight of Restorative Nurse Program
- On call rotation
NON-PERFORMANCE REQUIREMENTS:
Mission: Must know and appreciate the Mission of Incarnate Word Retirement Community.
Knowledge: Must be knowledgeable of state mandated M.D.S and its completion. Must have computer skills and PCC experience.
Accuracy: Must be able to maintain consistent work performance.
Physical Demands: Sits, walks, bends, stoops, and lifts at least 50 pounds throughout the eight-hour shift. Also, handing, pushing, pulling, carrying, reaching overhead, reaching forward, reaching low, kneeling, climbing and balancing.
Infection Control: Demonstrates knowledge of infection control in all residents care to prevent the spread of disease and infection. Adhere to strict infection control guidelines.
Safety: Demonstrates knowledge of emergency policy and procedure. Perform all responsibilities in a safe manner. Follow established guidelines when using protective devices and equipment. Demonstrates awareness of potential hazards and is practice in maintain a safe environment for residents and staff.
Communicating Resident’s Care: Utilizes plan of care in the delivery of resident’s care. Communicates and cooperates with other members if multi-disciplinary team to accomplish plan of care.
Resident relations: Perform all responsibilities with respect of interpersonal skills. Is pleasant, respectful and courteous in all interactions with resident’s families, staff, and visitors to the facility. Acts as representative of the Incarnate Word Retirement Community.
Professional Relations: Must be professional in performance if duties, dress and appearance.
JOB RELATIONSHIPS:
Responsible to: Director of Nurses.
Variables: May be required to work rotating shifts and/or rotating shifts and/or rotating units. May work Saturdays, Sundays, religious and other holidays.
